How to use this guide
Starting a New Medicine: The Keep It Together Care New Medicine Checklist
This checklist helps you gather the full list of everything the person takes, take the right questions with you before a new medicine starts, get the list checked by a pharmacist or prescriber, and keep a record of every change. It doesn't say whether a medicine is safe, whether medicines mix, or what to take.
On a phone, tablet or computer
Fill it in by typing
- Download the PDF from your email or from the download page.
- Open it in a PDF app that supports forms, such as Adobe Acrobat Reader (free on computers, phones and tablets).
- Tap a box and type. Tick boxes tick with one tap.
- Save the file. Your answers stay in your copy; nothing is sent to Keep It Together Care.
On paper
Print it and write by hand
- Print it on A4. The rows and boxes are sized for handwriting.
- Copy names and strengths exactly as they appear on the box or label.
- Keep it with the medicines, and take it to appointments and to the pharmacy.

Common questions
- Do I need to fill in every page?
- No. Start with the list on page 4 and the questions on page 6. Use the rest when you need it, one step at a time.
- What should go on the list?
- Everything the person takes, including things bought from a shop or pharmacy, vitamins and minerals, herbal products, creams, eye drops, inhalers, patches and injections. The checklist on page 5 helps you find the ones that are easy to forget.
- Who should I show the list to?
- A pharmacist, the GP or the team prescribing the new medicine. They can check how medicines mix. The checklist only helps you gather the full picture for them.
- I typed into it, but my answers had gone when I opened it again. Why?
- Some previews, such as a browser tab or an email preview, can't save answers in a form. Download the PDF first, open it in a PDF app that supports forms, such as Adobe Acrobat Reader, then save.
- Is anything I write sent anywhere?
- No. The PDF doesn't send your answers to Keep It Together Care or to anyone else. They stay in the file on your device, or on your paper copy.
